Enroll Your Child for the YouthQuest After School Program

To enroll your child for YouthQuest, you have two options:

Option 1: Download, complete and print.

Download, complete and print the registration form. (You can complete the form in PDF format on your computer, or you can print and write-in your answers.) Once you complete the form, return it to your child's school. (Download the registration form here.)


Option 2: Request a form by mail.

Submit a request (see right) to have the registration form mailed to you. You can then complete the form and return it to your child's school.

If you select option 2, please complete the request form to the right. We will then mail you a registration form. (NOTE: You need to complete one registration form for each child. If you need multiple forms, please select the correct number of children.)
